About your Medication Record Notebook
  • - Please bring your Medication Record Notebook with you at every visit: If you are a new patient, the Medication Record Notebook provides the details of your current medication (name and dosage of medicines), which should prevent prescription overlaps and unwanted drug interactions to seek out optimal remedies. This also helps you feel reassured about the treatment you receive.
  • - If you are a returning patient, it is very important for your doctor to check the medicines prescribed at any other medical institutions to ensure proper care.

1) Before Making an Appointment

Those with Japanese National Health Insurance
  1. For those with a referral from another medical institution in Japan
    When you wish to make an appointment, please call our appointment center, or for English speaker, contact the International Affairs Office as shown below.
  2. For those without a referral from another medical institution in Japan
    When you do not have a referral from another medical institution in Japan, the additional fee of 7,700 JPY (tax included) will be charged as a "fee for treatment of patients' choice", in addition to the first visit fee. When you wish to make an appointment, please call our appointment center, or for English speaker, contact the International Affairs Office as shown below.
Those without Japanese National Health Insurance
  1. You are asked to pay 20 yen per 1 medical remuneration point. When you do not have a referral from another medical institution in Japan, the additional fee of 7,700 JPY (tax included) will be charged as a "fee for treatment of patients' choice", in addition to the first visit fee. For those with travel insurance, we do not accept payment through travel insurance; you need to pay your medical expenses to the hospital at first and then claim these costs to your insurance companies later. We recommend you to bring a credit card with you. Moreover, please make sure to bring your passport with you. When you wish to make an appointment, please call our appointment center, or for English speaker, contact the International Affairs Office as shown below.

    The following credit cards can be used: VISA/MasterCard/JCB/AMERICAN EXPRESS/Diners Club/DC/NICOS/UFJ/UnionPay/Debit cards
    * A debit card is a payment card that deducts money directly from your bank account immediately (or a few days) after it is used. You can use your bank card with a debit card function and enter your PIN code to make a payment (Some cards cannot be used at our hospital; for more details, please ask your bank or our staff at the reception counter).

2) At the First Visit

What to bring with you
  • Japanese health insurance card (including beneficiary certificate and certificate of medical subsidies, if any)
  • A referral (from your current medical institution in Japan; Shinryo Joho Teikyosho): New patients without a referral will be charged 7,700 JPY (incl. tax) as a "fee for treatment of patients' choice", in addition to the first visit fee.
  • Photo identification (residence card or passport)
  • Medication Record Notebook

* We need information on your nationality and language for statistical purposes, and ask for your kind understanding and cooperation.

On the day of your first visit

Please fill out the Patient Registration Form placed on the desk at the General Waiting Lounge on the first floor and submit it, along with your Japanese medical insurance card, referral and photo identification, at the Reception Counter.

Interpretation services

When you have no good command of Japanese, we offer a paid interpretation service for English and Chinese. If you need the service, please let us know when you make an appointment. For other languages, we ask you to arrange a medical interpreter on your own.

Medical certificate in English

We can issue a Medical Certificate in English. (9,350 JPY per sheet, tax included).
For claim forms other than our hospital forms such as private insurance claim forms, please contact the International Affairs Office for document fees.
For documents in language other than English, please consult International Affairs Office.

For those seeking after-hours consultation
  1. If you need to have a consultation outside of regular hours and/or on Sundays/Holidays, please give us a call beforehand.

  3. Please make sure to bring your photo identification (such as residence card or passport) with you.
  4. A deposit of 10,000 JPY is required for those with Japanese National Health Insurance, or 50,000 JPY for those without Japanese National Health Insurance.
  5. If you do not have a referral from another medical institution in Japan, an extra fee of 7,700 JPY (tax included) will be charged as a "fee for treatment of patients' choice".
  6. We do not accept payment through travel insurance; you need to pay your medical expenses to the hospital at first and then claim these costs to your insurance companies later.
  7. We recommend you to bring a credit card with you.

* Please note that priority is given to patients with an appointment and thus those without an appointment may have to wait for a longer period of time; some departments may not accept walk-in patients.

Important Notice for Medical Treatment

  1. When having no request in which department to see a doctor, let us know your most severe symptom. If the symptoms are not reported correctly, it would take more time to refer you to the appropriate department.
  2. Arrange a professional medical interpreter in advance if you has no good command of Japanese. Doctors make diagnoses through the patient's complaints. If any misunderstanding occurs along the way, it could lead to a misdiagnosis. The use of specially-trained professional medical interpreter is indispensable. If you do not have any information to arrange your interpreter, please contact the International Affairs Office shown below.
  3. Medical consultation with a non-professional interpreter (your family members/relatives/company colleagues who understand Japanese) is not recommended. The intervention of non-professional interpreter may bring various disadvantages during the course of the medical treatment. In hospital setting, unfamiliar medical terms are frequently used, which requires sufficient expertise for accurate interpretation. In addition, it would pose psychological burden on a person who needs to function as a communicator without professional training. Furthermore, the disclosure of your information on illness may sometimes compromise your interests.
  4. For returning patients: when cancellation is needed, please be sure to call us before the scheduled appointment day.
  5. Bring your “Medication Record Notebook” (Okusuri Techo) without fail, when visiting the hospital.
  6. Please ask us if you encounter any unfamiliar words; difficult medical terms during the consultation can be paraphrased in a simpler expression.
  7. Please make sure any information that you thinks is important for your treatment by asking our medical staff without hesitation.
  8. When receiving informed consent form (for tests/treatments), please read it thoroughly. Only after totally agreeing with the content, you can sign the form.
  9. Some medical examinations require dietary restrictions or time limit from the eve of the test day. If any, please follow the instruction; otherwise you may not be able to have the test.
  10. Metallic accessories should be removed in X-ray/MRI scan. When you have a tattoo, let us know in advance.
  11. Avoid driving after the test, when instructed.

Dos and Don'ts in the Hospital Premises

  1. Covering natural facial/nail color can interfere with accurate diagnosis. Please refrain from wearing perfume and makeup.
  2. Please wear surgical masks in the hospital to prevent infection. Your understanding and cooperation would be appreciated.
  3. Do not speak loudly and wait quietly in the hospital public places such as hallways and waiting rooms, as with so many patients who feel sick out there.
  4. Do not run inside the hospital building. It will increase the fall risks among patients with a wheel chair/walker/cane/IV pole, which sometimes cause fracture.
  5. Smoking and drinking are prohibited in the hospital premises. When cigarette smoke is sensed in the washroom, sprinklers there will go off.
  6. Do not spit on the hospital hallways.
  7. When using a washroom, use toilet paper equipped there and flush it in the toilet.
  8. Set your mobile phone in silent mode. Talking on the phone is allowed in the designated space or outside of the building.
  9. Photo/video shooting is prohibited in the hospital, unless previously approved.
  10. We declines to provide medical services for those with violent behaviors (i.e. verbal abuse towards our staff) that could impair the patient-staff relationship. If the behavior is so extreme and out of control, we may report it to the police.
