About your Medication Record Notebook
  • - Please bring your Medication Record Notebook with you at every visit: If you are a new patient, the Medication Record Notebook provides the details of your current medication (name and dosage of medicines), which should prevent prescription overlaps and unwanted drug interactions to seek out optimal remedies. This also helps you feel reassured about the treatment you receive.
  • - If you are a returning patient, it is very important for your doctor to check the medicines prescribed at any other medical institutions to ensure proper care.

1) Before Making an Appointment

Those with Japanese National Health Insurance
  1. For those with a referral from another medical institution in Japan
    When you wish to make an appointment, please call our appointment center. If you have difficulty communicating in Japanese, please ask for the International Affairs Office when you make a call.
  2. For those without a referral from another medical institution in Japan
    When you do not have a referral from another medical institution in Japan, the additional fee of 7,700 JPY (tax included) will be charged as a “fee for treatment of patients’ choice”, in addition to the first visit fee. When you wish to make an appointment, please call our appointment center.
    If you have difficulty communicating in Japanese, please ask for the International Affairs Office when you make a call.

    ※The fee for treatment of patients’ choice will be 11,000 JPY (tax included) from April 1, 2025.

Those without Japanese National Health Insurance
  1. As your medical treatment is not covered by Japanese health insurance, it will be charged in accordance with our hospital regulations. Even if it is covered by overseas travel insurance, you still have to pay the whole bill by yourself. Please file your own claim with your insurance company at a later date. Credit cards are accepted for payment. Please bring your passport on the day of your visit. When you wish to make an appointment, please call our appointment center. If you have difficulty communicating in Japanese, please ask for the International Affairs Office when you make a call.

    The following credit cards can be used: VISA/MasterCard/JCB/AMERICAN EXPRESS/Diners Club/DC/NICOS/UFJ/UnionPay/Debit cards
    * A debit card is a payment card that deducts money directly from your bank account immediately (or a few days) after it is used. You can use your bank card with a debit card function and enter your PIN code to make a payment (Some cards cannot be used at our hospital; for more details, please ask your bank or our staff at the reception counter).

2) When You Visit the Hospital

What to bring with you
  • Japanese health insurance card (including beneficiary certificate and certificate of medical subsidies, if any)
  • A referral (from your current medical institution in Japan; Shinryo Joho Teikyosho): New patients without a referral will be charged 7,700 JPY (incl. tax) as a "fee for treatment of patients' choice", in addition to the first visit fee.
  • Photo identification (residence card or passport)
  • Medication Record Notebook

* We need information on your nationality and language for statistical purposes, and ask for your kind understanding and cooperation.

What to bring
  • Individual Number Card (My Number Card)

    ※If your health insurance card is not linked to your individual number card, please bring your health insurance card as well.

  • A referral from another medical institution in Japan

    ※If you do not have a referral letter, the additional fee of 7,700 JPY (tax included) will be charged in addition to the first visit fee.

    ※The fee for treatment of patients’ choice will be 11,000 JPY (tax included) from April 1, 2025.

  • Residence card or passport
  • Medication record notebook
Interpretation services

When you have no good command of Japanese, we offer a paid interpretation service for English and Chinese. If you need the service, please let us know when you make an appointment. For other languages, we ask you to arrange a medical interpreter on your own.

Medical certificate in English

We can issue a medical certificate in English. (8,800JPY per sheet, tax included).
If you wish to have documents prepared on a form other than ours (i.e., a specific form from an insurance company, etc.), please contact the International Affairs Office. If you wish to prepare documents other than in English, please contact the International Affairs Office.

For those seeking after-hours consultation
  1. Medical services are available only in Japanese and English.
  2. Please make sure to bring your photo identification with you.
    • residence card: if you are a Japanese residence
    • passport: if you are a traveler
  3. Since the accounting section closes after-hours, we require a deposit.
    • 10,000JPY : if you have a Japanese Health Insurance
    • 100,000JPY:if you do not have a Japanese Health Insurance
  4. If you have a Japanese Health Insurance and do not have a referral from another medical institution in Japan, and extra fee of 7,700JPY (tax included) will be charged as a “fee for treatment or patients’ choice”.

    ※The fee for treatment of patients’ choice will be 11,000 JPY (tax included) from April 1, 2025.

  5. Even if your medical fees are covered by overseas travel insurance, you still have to pay the whole bill by yourself. Please file your own claim with your insurance company at a later date.
