Procedure for Outpatient Consultation

For New Patients

  • Please complete the Patient Registration Form from the desk at the General Waiting Lounge on the first floor and submit the form, along with your Medical Insurance Card, at the Reception Counter.
  • If you have been referred to our hospital from another medical institution, please submit the referral letter as well.

For Returning Patients

  • Please use Self-Registration Kiosks located at the right side of the Main Entrance in the Hospital Building for check-in.
  • After inserting your Hospital ID Card, follow the instructions on the touchscreen to confirm your visit and retain the reception slip and the numbed slip that are issued from the machine. Please take these slips and put them in a clear file folder already prepared on the side table next to the machine.
  • If you are late for your appointment, are visiting for the first time of the month, or there has been a change to your insurance card or address, please visit the Reception Counter. If you have any questions, please do not hesitate to ask our staff.

After Hours Consultation

  • If you need to have a consultation outside of normal hours, please call before you visit.
  • If you do not have a referral letter from another medical institution, you will be required to pay an additional expense of 5,500 JPY (incl. tax) in most cases.

Procedure before Consultation (after check-in)

  • After the check-in procedure on the first floor, please go directly to the relevant department and give your clear file folder (with the reception slip inside) to a clerk at the counter. Please wait until your number is displayed on the screen.
  • If you need to have any examinations (such as blood/urine tests and imaging study like X-ray) before consultation, please go to the corresponding examination room first to have these tests before the consultation. Please note that for some examinations, you may need to visit the relevant department before the examination.
  • If you have appointments with more than one department, please first visit the department for which you have the earliest appointment.

Procedure following Consultation (Payment)

  • After your consultation, please give the clear file folder to the staff at the Cashier Counter.
  • Your number is displayed on the monitor screen when your bill is ready. Please pay at the Cashier counter or use the Automated Cashier Machine.

For Patients who need to fill a Prescription

  • A voucher for medicine will be attached to your receipt when you pay your bill.
  • Please go to the pharmacy inside the hospital. Please exchange the voucher for your medication when the number on the voucher is displayed on the monitor.
